The Joint Commission has initiated the Flu Vaccine Challenge for 2008.
Please look at your paycheck this week for the Challenge, locations for low cost or no cost flu shot clinics.
Please sign the back of the form, stating if you have had the Flu Vaccine or to decline to Vaccine and return it to my office.
